The distinguished PCAST Working Group on Patient Safety was co-led by PSMF Founder Joe Kiani and Microsoft’s Chief Scientific Officer, Eric Horvitz. Just a few days before on September 6, the Anesthesia Patient Safety Foundation (APSF) held its Stoelting Conference. More than 40 percent of these events are due to preventable errors.
