Humana launches in-home primary care through CenterWell

Humana's senior-focused primary care business, CenterWell, is bringing its services into patients' homes.

The Medicare Advantage insurer announced Wednesday that CenterWell will offer in-home primary care to seniors who receive care at select locations in Georgia and Louisiana. The new Primary Care Anywhere program harnesses the capabilities of Heal, which Humana acquired earlier this year.

The program will initially launch for patients living in Atlanta, Savannah and Columbus in Georgia as well as Baton Rouge and New Orleans in Louisiana, the insurer said. Patients will be able to receive a number of services traditionally performed in the doctors' office, such as blood draws, vaccinations and prescription management, in the more convenient home setting.

Primary Care Anywhere deploys a team of board-certified clinicians, and patients will pay the same copayments for services rendered in the home or in the clinic.

“We know that improving health outcomes goes beyond traditional clinical settings,” said Vivek Garg, M.D., chief medical officer for CenterWell Senior Primary Care, in the release. “When we meet patients where they are, we build better connections and can tailor their care to address environmental factors that impact their health."

"Primary Care Anywhere brings the comprehensive, whole-person care we provide in our centers to patients at home," he added. "They can expect the same integrated care teams who keep them at the center of everything we do.”

Patients with a wide array of Medicare Advantage plans, as well as those enrolled in traditional Medicare, can receive services from CenterWell, including those provided through Primary Care Anywhere.

CenterWell and its sister brand Conviva Care Center treat more than 272,000 seniors at over 250 centers in 12 states. Humana is also planning to continue significant expansion, adding between 30 and 50 new centers each year through 2025.

Renee Buckingham, president of CenterWell Senior Primary Care, said in the release that continuing to expand into new avenues like home health reflects Humana's commitment to providing omnichannel care options to its members and patients.

“Our integrated care model is unique,” Buckingham said. “Our multi-disciplinary team approach allows our clinicians to spend more time with our patients to address their acute, chronic and social determinant needs—both in our centers and in patients’ homes."
