On March 30, 2023, the HHVBP Model Technical Assistance (TA) Team hosted the live streaming event, Strategies for Success Self-Assessment Tool. During this event, the TA Team provided home health agencies (HHAs) with an introduction to a quality improvement self-assessment and planning tool that HHAs can use to identify and prioritize performance improvement initiatives. The self-assessment tool presented during this event can be used for evaluating the application of nine (9) recommended best practices in the areas of assessment, care planning, monitoring, patient engagement, and maintenance coverage.
