On July 25, 2023, CMS posted QSO-23-02-ALL … EXPIRED EFFECTIVE: June 5, 2023.
The Staff Vaccination Requirements have expired for all providers:

- Rural Health Clinic/Federally Qualified Health Center (RHC/FQHC) Attachment Expired 6/5/2023
- End-Stage Renal Disease (ESRD) Facility Attachment Expired 6/5/2023
The only provider that does not specifically state that the staff vaccination requirements have required is:
Intermediate Care Facilities for Individuals with Intellectual Disabilities (ICF/IIDs) Attachment Revised (page 60 of the 163-page Memorandum).
I suspect that this is an inadvertent error – CMS just missed tagging it as expired for ICF/IIDs. You may want to check with your survey agency to be sure.
