Project ECHO Series: Long-Term Care Medication Management of Blood Thinners

(Telligen’s) goal is to provide the same opportunity to improve the high-risk medication management system with the two different provider groups (nursing homes & prescriber/pharmacists) with each their own ECHO series. A certificate of completion will be provided to individuals who attend all the sessions in the series. Please share the following with your nursing homes.

With the focus on anticoagulant/blood thinner medications we would appreciate if you and your nursing home staff joined us in our Project ECHO® Series: Long-Term Care Medication Management of Blood Thinners. This is a four-week series of 30-minute sessions (session 1 is 45 minutes and sessions 2-4 have the option of staying on an additional 15 minutes at the end for open discussion). We are excited to bring you subject matter experts, recommendations, tools, resources and build on collaboration efforts during our time together. In return, we ask you share your experience during the case study and open discussion as well as move forward in action with anything you gain during the sessions. Some benefits that may result by attending this series include building communication and collaboration efforts across settings, prevention of adverse drug events (ADEs) and/or the devasting results to the resident who experiences an ADE, impact rehospitalizations and ED visits, Five-Star QM rating system, medication management systems, improve symptoms associated with bleeding or clotting events, quality improvement processes, gain new resources, tools, and subject matter expert knowledge sharing, support training new team members and refreshing seasoned staff, network together to mitigate challenges and share successes and the potential of so much more.
