Humana’s CenterWell Launches At-Home Primary Care Program

CenterWell Senior Primary Care – a part of Humana Inc. (NYSE: HUM) – is expanding into the home.

It announced Wednesday that, in select Georgia and Louisiana locations, patients would be able to receive at-home primary care services under its “CenterWell Primary Care Anywhere” program.

CenterWell is Humana’s senior care services arm. It includes pharmacy, primary care and home health services. CenterWell Home Health came out of Humana’s acquisition of what used to be Kindred at Home, the largest home health provider in the country.

CenterWell Primary Care’s entrance into the home is consistent with Humana’s longstanding effort to drive more home- and value-based care.

“We know that improving health outcomes goes beyond traditional clinical settings,” CenterWell Senior Primary Care Chief Medical Officer Dr. Vivek Garg said in a statement. “When we meet patients where they are, we build better connections and can tailor their care to address environmental factors that impact their health. Primary Care Anywhere brings the comprehensive, whole-person care we provide in our centers to patients at home. They can expect the same integrated care teams who keep them at the center of everything we do.”

CenterWell Senior Primary Care delivers care to more than 272,000 seniors in over 250 centers across 12 states. It hopes to add 30-50 new centers per year through 2025.

The Primary Care Anywhere program was driven by Humana’s acquisition of the at-home primary care provider Heal in January. Humana first invested $100 million in Heal in 2020.

A team of clinicians will ensure patients receive routine primary care services they’d “normally get in a doctor’s office,” according to a press release. Those services include: blood draws, vaccinations and prescription management, among others.

CenterWell’s Primary Care Anywhere will accept patients from a variety of Medicare Advantage plans, as well as those with traditional Medicare.

“One of the areas we’re very excited interested in is primary care at home,” Dr. Andrew Agwunobi, the president of Humana’s home solutions business, said at Home Health Care News’ FUTURE event Wednesday. “We just announced this morning CenterWell Primary Care Anywhere. We’re starting off in Georgia and Louisiana, and we’re very excited about that [model].”

All signs point to CenterWell expanding primary care at home – one of the fastest-growing at-home care service lines – across the country.

Previously, Humana leaders have also touted senior primary care as a way to drive home health utilization for CenterWell.
