State Health Care Transformation Initiatives 

With support from Arnold Ventures, the National Academy for State Health Policy (NASHP) is collaborating with leading states in their efforts to transform their health systems by pursuing payment and delivery system reforms through population-based global budgets, value-based payments, and investments in primary care to help ensure patient access to high quality care.  

Many states are looking to move away from the health system’s current reliance on fee-for-service, utilization-driven payment and are pursuing reforms that incentivize improving population health and provide predictable funding to providers in ways that also contain costs. This project aims to assist states in progressing their unique health system transformation efforts toward the overarching goals of better health access and outcomes and minimized cost growth. Some states are pursuing Centers for Medicare & Medicaid Services (CMS) Innovation Center models, such as the States Advancing All-Payer Health Equity Approaches and Development (AHEAD) and Making Care Primary (MCP) Models, while others are designing state-specific initiatives.  

NASHP will document and disseminate lessons learned from state policy approaches, as well as gather interested parties in strategic state-only convenings. This toolkit serves as a repository for relevant materials and resources. 

Transforming the Health System

A growing number of states are exploring innovative payment reforms that move away from a utilization-driven, fee-for-service system to approaches that incentivize improved health outcomes and provide more predictable budgets to health care providers. To achieve these goals, states are engaging their partners within their health markets, crafting new policies to support shifting payment designs, developing meaningful quality measures that can be supported by enhanced data infrastructures, and more. 

State Engagement with Federal Models

Two of the CMS Innovation Center models — the AHEAD and MCP Models — share some of states’ transformation goals. Examples include moving toward capitated payments, prioritizing primary care, and focusing on broad population health goals. These resources are developed for states to explore the potential opportunities within these models and to understand what may be learned from them.
